I opened the official itch.io embed on 23 October 2026. Two notices came up first — a thanks-for-playing note for v1.19, then a Story-mode playtest invite — and only then the menu. In Sandbox the pistol was already selected. One click on the forearm logged six wounds, dropped blood from 5000 mL to 4994 mL, and set pain to 37. The hit label read that a 9×19 mm FMJ round had lodged in the right hand.
What this game is
Trauma Lab, styled Trauma lab on the itch.io page, is a simulation by the itch author TraumaLab. The description is signed Abynotic Studios. The line on the page is: an anatomy and physics sandbox where every bullet is tracked through skin, muscle, bone and organ.
The public build is v1.19. itch.io lists the release at 20 October 2026, 15:06 UTC, and the last update at 22 October 2026, 16:44 UTC. Status is In development. It is free, with name-your-own-price, plus a $5 Supporter tier. Platforms listed are HTML5, Windows, and Android. Tags include anatomy, browser, Gore, medical, Physics, ragdoll, Sandbox, Singleplayer, and Violent. The page also marks the project AI Assisted for code, graphics, and sounds.
Three ways to start
The menu offers Sandbox (every weapon, tool, chemical, and power), Keep Him Alive (ten emergency cases, medicine only), and Story, which is marked prototype and locked on the free build. Surgery can be Simple, which finishes steps for you, or Complex, which asks you to use the scalpel, saw, clamps, and sutures yourself.
The launch screen states 18+, graphic violence and gore, and that this is a fictional simulation, not medical advice. The browser copy embedded here is the official itch upload 19947427. This site does not host the game files.
Questions
Is Trauma Lab free?
Yes. The itch.io page is name-your-own-price, including zero. A $5 Supporter tier is optional and lists a newer playtest file.
Is this a hospital trauma lab?
No. Trauma Lab here is the Abynotic Studios sandbox. University labs and blood-test panels that share the name are a different thing.
