Trauma Lab Review — Hands-On Notes from v1.19
I played one Sandbox session in the official embed and stopped there. I did not finish a Keep Him Alive case, and Story was locked, so this is not a review of the whole game. The load put two dialogs in front of the menu: thanks for playing v1.19, then an invite to the Story playtest. That friction is real. Once the menu was up, Simple versus Complex surgery and the 18+ line were the choices that mattered.
What the pistol shot showed
The default tool was the pistol. One click marked the subject Injured, listed six wounds, moved blood from 5000 mL to 4994 mL, and set pain to 37. The label named the right forearm and a bullet lodged in the right hand. X-ray with Layers and Examine kept the HUD up: heart rate 79, blood pressure 122/78, oxygen 98, blood 4994. The feedback is specific in a way most ragdoll sandboxes are not.
Options on that build include gore from Mild to Extreme, quality Auto / High / Low, six gravity settings, god mode, bleed-out, reactions, pain faces, arrow cam, and slow motion. The first-run gore default is Extreme.
What I would not claim
itch.io showed 4.9 from 21 ratings, with a tooltip of 4.86, on 23 October 2026. That is their number, not a score from this site. Comments praised the gore and compared the feel to People Playground. Other comments asked for more gore, fullscreen, and a way to sedate the subject, and one reported a black screen on Android. I cannot weigh those against a single session.
The rough edges I did hit: two popups before play, no written manual, Story locked, a phone-sized embed with a lot of empty margin, and a Windows download that is an HTML file while the banner says exe. v1.20 is opaque because its changelog is not public.
Questions
What score does this page give Trauma Lab?
None. The only rating cited is itch.io’s own 4.9 from 21 ratings on 23 October 2026.
Did you finish Story mode?
No. The free build locks it. The notes cover one Sandbox session.
